Abstract
Surabaya City Health Office is the government agency that is engaged in the field of health. As government agencies are implementing timeliness in reporting each health center, Surabaya Health Department often experience delays in monitoring and evaluating for some periods, so that the figure has decreased especially in the health program Dengue Hemorrhagic Fever (DHF), are also experiencing delays in handling cases , which is due to the delay in reporting the health centers and health center locations remote from the health department. From the analysis, it is known that the delay in reporting health centers that do result in additional cases. From the analysis of the issues and problems that exist, then the application needs to perform the monitoring and evaluation of web-based dengue control which is built based on interviews and observations that have been made. By using the web-based may help the health department and community health center in Surabaya in addressing issues of time efficiency of reporting and control and to speed up the evaluation process so that morbidity and mortality can be reduced as low as possible. Based on the results of testing and evaluation, it is known that the application can provide evaluation results in a month, so it will not happen Extraordinary Events (KLB), the percentage of morbidity indicators (IR) 1.4 / 100,000 population, the mortality rate (CFR) 0 % and number-free larvae (ABJ) 85%.
